CheckMarq Forms

Some forms are available to active students in electronic format within CheckMarq.

How to Submit a New Form

1. Navigate to CheckMarq Student Home > Tasks tile > Forms

2. Click or tap on the form you want to complete from the left-hand navigation menu

3. Complete the form.  If you need to save the form and return to it later, click or tap the "Save" button on the bottom of the form.

4. When ready to submit, click or tap the "Submit" button on the bottom of the form.

5. After submitting your form, you will be taken to a confirmation page letting you know that your form has been submitted.  You may click or tap on the Home icon to return to CheckMarq Student Home, or exit your browser.  You will receive an email within a few minutes confirming that your form has been received.

How to Access a Saved Form

  1. Navigate to CheckMarq Student Home > Tasks tile > Forms.
  2. Select Update a Saved Form from the left-hand navigation menu.

  3. Click or tap the Search button. Optionally, you can use the search fields to narrow your results. The search will return any form that you have saved, that you have submitted but is still pending approval by someone (eg: your college records office), or that an approver has recycled so that you can edit and resubmit it.
  4. Select the saved form you want to open.

  5. Your form will open. Continue editing it and use the buttons at the bottom of the form to save or submit it when ready.
  6. f you no longer intend to submit your form, click the Withdraw button on the bottom of the form to remove it from your saved forms.

How to View and Check the Approval Status of a Previously Submitted Form

  1. Navigate to CheckMarq Student Home > Tasks tile > Forms.
  2. Select View a Submitted Form from the left-hand navigation menu.

  3. Click or tap the Search button. Optionally, you can use the search fields to narrow your results.
  4. The Form Status column will tell you the current form status. Here are some common Form Statuses you may see:
    • Denied: Your form was submitted and has been denied by an approver.
    • Executed: You form has been submitted and approved by any necessary approvers.
    • Pending: Your form has been submitted and is pending approval.
    • Recycled: An approver has sent your form back to you to make changes. Search for it in 鈥淯pdate a Saved Form鈥 to edit and resubmit it.
    • Saved: Your form is saved but not yet submitted. Search for it in 鈥淯pdate a Saved Form鈥 to complete and submit it.
  5. Click on a form to open it and view additional details.

  6. Your form will open. Use the Next button on the bottom of the form to navigate to the last page.

  7. The last page displays the current form status in the upper right-hand corner of the form.
  8. If the form status is 鈥淧ending,鈥 you can click on the View Approval Route button to see which approval step is pending.
  9. 鈥淭he Transaction/Signature Log鈥 lists every time you or an approver takes action on the form.

Available CheckMarq Forms

Change or Declare Major/Minor

This form is available to undergraduate students who need to change or declare a major or minor. 

MCC Discovery Theme

This form is available to undergraduate students who need to declare or change their Marquette Core Curriculum (MCC) Discovery Theme.  See How Do I Declare or Change My Marquette Core Curriculum (MCC) Discovery Tier Theme? for more information.

Release of Education Records

This form is available to all students to authorize Marquette personnel to release a portion of their educational records to a third party. See How Do I Authorize or Revoke the Release of Education Records to a Third Party? for more information.

Transfer Course Request

This form is available to undergraduate students who are seeking approval to transfer coursework from another US institution. 

Former students who do not have access to CheckMarq should use the Transfer Course/Credit Request for former undergraduate students seeking readmission PDF form.
